Use this form to search our extensive database of US1-area health resources. You may search based on partial name matches, phone numbers and/or categories.

Categories with no subheadings are shown as checkboxes. Categories with subheadings are shown as list boxes. These list boxes allow multiple selections. Most browsers support multiple selections through holding Shift for contiguous selections and Control for non-contiguous selections. If you inadvertently select a subheading and your browser does not allow you to unselect the entire category, you may select None, which will cause the search engine to ignore the category. Selecting All will force the search engine to treat the category as if all subheadings were selected.
